Who is Eligible for Coverage?

A customer of who maintains an active bank account.

A customer also may apply for coverage for a Spouse and for a Dependent Child.  A Dependent Child is a biological child, a stepchild, an adopted child, and any child for whom the customer is required by a court or administrative order to provide health coverage who is unmarried and less than 26 years of age and any child who, regardless of age, is incapable of self-sustaining employment due to mental retardation or physical handicap and is dependent upon the customer for support, provided that proof of such incapacity is provided to the Company within 31 days following the child’s 26th birthday and at the request of the Company not more frequently than annually thereafter.

Coverage Provided:

The Company will provide benefits in one lump sum if a covered accident results in an injury listed below or death within 365 days of the covered accident.  The amount of the benefit provided will be reduced to 50% of the amount listed below when the person who is injured in, or dies as a result of, the accident reaches Age 70 and to 25% of the amount listed below when that person reaches Age 75.  The premiums for the insurance will not be reduced.

A customer who accepts the basic coverage by signing and returning the Activation Form will receive $3,000 in coverage for one year from the Coverage Effective Date.  A customer may apply for Additional Coverage of up to $300,000 for the customer or for the customer’s Spouse and Dependent Children by completing and signing and returning the Enrollment Form.  Note that no more than one benefit will be payable for all losses to the same limb due to or related to any one accident; the benefit payable in this circumstance will be the largest benefit payable.
