The additional benefits below are provided only if the customer applies for and is issued Optional AD&D coverage. If Family coverage is applied for and is issued, the amounts of such coverage will be:
Family Benefit amounts:
- Spouse/Registered Domestic Partner Only (no children): 60% of the benefits provided to customer
- Dependent Children Only (no Spouse/Registered Domestic Partner): 25% of the benefits provided to customer
- Spouse/Registered Domestic Partner and Dependent Children: Spouse/Registered Domestic Partner: 50% and Dependent Children: 20%
Exposure to the Elements & Disappearance Within One (1) Year
No more than your Amount of Insurance under this Coverage at the time of the accident will be paid for all Losses resulting from Injuries sustained in that accident. For the purposes of the Coverage:- Exposure to the Elements will be considered an accidental bodily Injury. Exposure to the Elements means exposure to severe hot or cold weather that results in actual significant physical injury including sun stroke, heat stroke and frostbite.
- It will be presumed that the person has suffered a Loss of life if the person’s body has not been found within one year of disappearance, stranding, sinking or wrecking of any vehicle in which the person was an occupant.
Automatic Increase in Member Amount of Insurance
On the first anniversary of the date member became insured under the Coverage, it will be increased by 2% of the original Base Amount of Insurance, followed by 2% increases on the 4 succeeding anniversaries to a maximum increase of 10% of the original Base Amount of Insurance.
Common Carrier Benefit
The plan pays an additional benefit equal to the lesser of: 100% of the Amount of Insurance on the person and $500,000.
Coma
The lesser of 2% per month and up to $1,000 up to 100 months; after 100 months, a lump sum equal to 100% of the Amount of Insurance minus the amount already paid for Coma.
Seat Belt Benefit
The plan pays an additional benefit equal to the lesser of: 10% of the Amount of Insurance on the person; and $10,000.
Airbag Benefit
The plan pays an additional benefit equal to the lesser of: 10% of the Amount of Insurance on the person; and $10,000.
Tuition Reimbursement for Your Dependent Spouse or Registered Domestic Partner
The plan pays an additional benefit equal to the lesser of: the actual annual tuition, 15% of the Amount of Insurance on the person and $15,000, payable annually for up to 4 consecutive years. Enrollment in that program must occur within 12 months of the member’s death. If there is no dependent Spouse/Registered Domestic Partner eligible for this benefit, a benefit of $1,000 will be paid.
Motorcycle Accident While Wearing Safety Equipment
An amount equal to the lesser of: 10% of the Amount of Insurance on the person; and $10,000.
Beneficiary
Loss of life benefits are paid to the person you designate as your beneficiary. Any amount of insurance for which there is no Beneficiary at your death will be payable to the first of the following: your (a) surviving Spouse/Registered Domestic Partner; (b) surviving child(ren) in equal shares; (c) surviving parents in equal shares; (d) surviving siblings in equal shares; and (e) estate. You may change your beneficiary at any time while you are alive. The beneficiary change form must be filed through your Contract Holder.
Certificate of Insurance
Prudential will make available to each customer a Certificate of Insurance which will reflect the coverages under the Policy.
Termination of Additional Insurance
All Additional Coverage will end on the earliest of the following dates:

- the date upon which the Policy terminates;
- the date upon which the 31-day Grace Period expires if no premium payment has been received.
Termination of coverage will not affect a claim based upon an accident that occurred prior to the termination of coverage date. Additional Coverage for a Spouse/Registered Domestic Partner and Dependent Children will terminate upon the earlier of the date upon which the Spouse/Registered Domestic Partner or Dependent Child is no longer eligible for coverage, or the date upon which the customer’s coverage ends.
Your Premiums
Optional Accidental Death and Dismemberment coverage is provided on a contributory basis. This means that you are responsible for paying the premium for your coverage. You will be informed of the amount of your premium payment when you enroll. You must pay each premium, other than the first, within 45 days of the premium payment date without being charged interest (known as the grace period). If you fail to pay any premium required by the end of the grace period, then member insurance under the Optional Accidental Death and Dismemberment coverage will end when the grace period ends and no benefits will be payable.
All decisions concerning the payment of claims under the plan are at the sole discretion of Prudential.
